RATIONALE AND OBJECTIVE: Treatment for head and neck cancer (HNC) can lead to decreased oral intake which often requires gastrostomy tube (g-tube) placement to provide nutritional support. A multidisciplinary team (MDT) consisting of interventional radiology (IR), HNC oncology and surgery, nutrition, and speech language pathology departments implemented an expedited outpatient g-tube placement pathway to reduce hospital stays and associated costs, initiate feeds sooner, and improve communication between care teams. This single center study investigates differences in complications, time to procedure and costs savings with implementing this pathway. METHODS: 142 patients with HNC who underwent elective image guided g-tube placement by IR from 2015 to 2022 were identified retrospectively. 52 patients underwent the traditional pathway, and 90 patients underwent the expedited pathway. Patient demographics, procedure characteristics, periprocedural costs and 90-day complication rates were collected and compared statistically. RESULTS: The 90-day complication rate was comparable between groups (traditional=32.7%; expedited=22.2%; p-value=0.17). The expedited pathway decreased the time from consult to procedure by 11.1 days (95% CI 7.6 - 14.6; p < 0.001) and decreased charge per procedure by $2940 (95% CI $989-$4891; p < 0.001). CONCLUSION: A MDT for the treatment of patients with HNC successfully provided enteral nutrition support faster, with fewer associated costs, and in a more patient centered approach than previously done at this institution.

BACKGROUND: Intensive lipid-lowering therapy may induce coronary atherosclerosis regression. Nevertheless, the factors underlying the effect of lipid-lowering therapy on disease regression remain poorly characterized. Our aim was to determine which characteristics of atherosclerotic plaque are associated with a greater reduction in coronary plaque burden (PB) after treatment with alirocumab in patients with familial hypercholesterolemia. METHODS: The ARCHITECT study (Effect of Alirocumab on Atherosclerotic Plaque Volume, Architecture and Composition) is a phase IV, open-label, multicenter, single-arm clinical trial to assess the effect of the treatment with alirocumab for 78 weeks on the coronary atherosclerotic PB and its characteristics in subjects with familial hypercholesterolemia without clinical atherosclerotic cardiovascular disease. Participants underwent a coronary computed tomographic angiography at baseline and a final one at 78 weeks. Every patient received alirocumab 150 mg subcutaneously every 14 days in addition to high-intensity statin therapy. RESULTS: One hundred and four patients were enrolled. Median age was 53.3 (46.2-59.4) years and 54 were women (51.9%). The global coronary PB changed from 34.6% (32.5%-36.8%) at entry to 30.4% (27.4%-33.4%) at follow-up, which is -4.6% (-7.7% to -1.9%; P<0.001) reduction. A decrease in the percentage of unstable core (fibro-fatty+necrotic plaque; from 14.1 [7.9-22.3] to 8.0 [6.4-10.6]; -6.6%; P<0.001) was found. A greater PB (ß, 0.36 [0.13-0.59]; P=0.002) and a higher proportion of unstable core (ß, 0.15 [0.08-0.22]; P<0.001) were significantly related to PB regression. CONCLUSIONS: Treatment with alirocumab in addition to high-intensity statin therapy might produce a greater PB regression in patients with familial hypercholesterolemia with higher baseline PB and in those with larger unstable core. La infiltración cutánea por células leucémicas conocida como leucemia cutis es una presentación infrecuente de esta patología y constituye un desafío diagnóstico. Los diagnósticos como infecciones, otras patologías neoplásicas con afectación cutánea y los trastornos histiocíticos, entre otros, constituyen los principales diagnósticos diferenciales, ya que configuran un escenario pronóstico y terapéutico diferente. Se presentan dos pacientes que fueron diagnosticados inicialmente como leucemia cutis, cuyo diagnóstico final fue de patologías no malignas.

BACKGROUND: The effect of alirocumab, a PCSK9 (proprotein convertase subtilisin/kexin type 9) inhibitor, on coronary plaque burden in patients with familial hypercholesterolemia has not been addressed. Our aim was to assess changes in coronary plaque burden and its characteristics after treatment with alirocumab by quantification and characterization of atherosclerotic plaque throughout the coronary tree on the basis of a noninvasive analysis of coronary computed tomographic angiography in asymptomatic subjects with familial hypercholesterolemia receiving optimized and stable treatment with maximum tolerated statin dose with or without ezetimibe. METHODS: This study is a phase IV, open-label, multicenter, single-arm clinical trial to assess changes in coronary plaque burden and its characteristics after 78 weeks of treatment with alirocumab in patients with familial hypercholesterolemia without clinical atherosclerotic cardiovascular disease. Participants underwent an initial coronary computed tomographic angiography at baseline and another at 78 weeks. Every patient received 150 mg of alirocumab subcutaneiously every 14 days in addition to high-intensity statin therapy. The main outcome was the change on coronary plaque burden and its characteristics by quantification and characterization of atherosclerotic plaque throughout the coronary tree on the basis of analysis of coronary computed tomographic angiography. RESULTS: The study was completed by 104 patients. The median age was 53.3 (46.2-59.4) years. Of these patients, 54 were women (51.9%). Median low-density lipoprotein cholesterol was 138.9 (117.5-175.3) mg/dL at entry and 45.0 (36.0-65.0) mg/dL at follow-up (P<0.001). Coronary plaque burden changed from 34.6% (32.5%-36.8%) at entry to 30.4% (27.4%-33.4%) at follow-up (P<0.001). A significant change in the characteristics of the coronary atherosclerosis was also found: an increase in the proportion of calcified (+0.3%; P<0.001) and mainly fibrous (+6.2%; P<0.001) plaque, accompanied by a decrease in the percentage of fibro-fatty (-3.9%; P<0.001) and necrotic plaque (-0.6%; P<0.001). CONCLUSIONS: Treatment with alirocumab in addition to high-intensity statin therapy resulted in significant regression of coronary plaque burden and plaque stabilization on coronary computed tomographic angiography over 78 weeks in these groups of patients with familial hypercholesterolemia without clinical atherosclerotic cardiovascular disease. In this work, a procedure to obtain an accurate value of the critical speed of a cracked shaft is presented. The method is based on the transversal displacements of the cracked section when the shaft is rotating at submultiples of the critical speed. The SERR (Strain Energy Ralease Rate) theory and the CCL (Crack Closure Line) approach are used to analyse the proposed methodology for considering the behaviour of the crack. In order to obtain the best information and to define the procedure, the orbits and the frequency spectra at different subcritical rotational speed intervals are analyzed by means of the Fast Fourier Transform. The comparison of the maximum values of the FFT peaks within the intervals allows the subcritical speed to be determined, along with the value of the critical speed. When verified, the proposed procedure is applied to shafts with the same geometry and material and with cracks of increasing depth. The results show that the critical speed diminishes with the severity of the crack, as expected. A comparison is made between the critical speed obtained using the vertical and the horizontal displacements, finding no remarkable differences, meaning that in practical applications only one sensor for one of the displacements (in the vertical or horizontal direction) is needed to determine the critical speed. This is one of the main contributions of the paper, as it means that the orbits of the shaft are not needed. Finally, after this study we can conclude that the best results are achieved when the critical speed is obtained using data displacement in only one direction within the intervals around 12 or 13 of the critical speed.

Introducción: la disfagia es un síntoma frecuente y grave en las personas con daño cerebral y se necesita una herramienta que permita cuantificar y objetivar su evolución. Objetivo: desarrollar y validar una herramienta que mida la evolución de la disfagia en un paciente con daño cerebral adquirido a fin de facilitar la toma de decisiones durante el tratamiento multidisciplinar. Métodos: estudio retrospectivo que consta de dos fases con el objetivo de desarrollar una nueva herramienta. En la primera se desarrolla el instrumento y en la segunda se estudian las propiedades psicométricas tras su aplicación a 83 pacientes, la validez de contenido, la validez convergente, la validez de grupos conocidos, la fiabilidad y la sensibilidad al cambio. Resultados: se obtuvo la escala EDMUR, un instrumento compuesto por 5 dominios y 46 ítems. Tras el juicio de expertos, todos los ítems propuestos en la fase de diseño de la escala alcanzaron los valores mínimos de V de Aiken y CRV´. El valor de la alfa de Cronbach (α) mostró una buena consistencia interna. La correlación entre la escala FOIS y la EDMUR, obtenida a través del coeficiente de Spearman (rho), fue directa y significativa. La prueba H de Kruskal-Wallis reveló diferencias estadísticamente significativas entre todos los grupos establecidos según los niveles de la FOIS. Para la sensibilidad interna se empleó la suma de rangos con signo de Wilcoxon, obteniéndose una diferencia significativa entre las dos valoraciones. La sensibilidad externa de la escala obtuvo una correlación directa y significativa. Conclusiones: la escala EDMUR es una herramienta fiable, válida y sensible para la cuantificación de la evolución de la disfagia, mejorando la calidad de los tratamientos.(AU)


Introduction: dysphagia is a frequent and serious symptom in people with brain damage and a tool is needed to quantify and objectify its evolution. Objective: to develop and validate an instrument to measure the evolution of dysphagia in a patient with acquired brain damage in order to facilitate decision-making during multidisciplinary treatment. Methods: a longitudinal, retrospective cross-sectional study that consists of two phases with the aim of developing a new tool. In the first phase the instrument is developed and in the second psychometric properties, after application to 83 patients, is analyzed together with content validity, convergent validity, validity of known groups, reliability, and sensitivity to change. Results: an instrument, the EDMUR scale, composed of 5 domains and 46 items was obtained. After expert judgment, all the items proposed in the scale design phase reached the minimum values of Aiken's V and CRV'. Cronbach's alpha (〈) showed a good internal consistency. The correlation between the FOIS scale and the EDMUR obtained through Spearman's coefficient (rho) was direct and significant. The Kruskal-Wallis H-test revealed statistically significant differences between all the groups established according to the levels of the FOIS. For internal sensitivity, the Wilcoxon signed rank sum test was used, obtaining a significant difference between the two assessments. The external sensitivity of the scale obtained a direct and significant correlation. Conclusions: the EDMUR scale is a reliable, valid, and sensitive instrument for quantifying the evolution of dysphagia, thus improving the quality of treatments (AU)

An association of deletions in the IKZF1 gene (IKZF1del) with poor prognosis in acute lymphoblastic leukemia (ALL) has been demonstrated. Additional deletions in other genes (IKZF1plus) define different IKZF1del subsets. We analyzed the influence of IKZF1del and/or IKZF1plus in the survival of children with ALL. From October 2009 to July 2021, 1055 bone marrow samples from patients with ALL were processed by Multiplex ligation-dependent probe amplification (MLPA). Of them, 28 patients died during induction and 4 were lost-in-follow-up, resulting in an eligible 1023 cases. All patients were treated according to ALLIC-BFM-2009-protocol. Patients were classified into three subsets: IKZF1not-deleted (IKZFF1not-del), IKZF1deleted (IKZF1del) and IKZF1del plus deletion of PAX5, CDKN2A, CDKN2B and/or alterations in CRLF2 with ERG-not-deleted (IKZF1plus). The LFSp and SE were calculated with the Kaplan−Meier calculation and compared with a log-rank test. From the 1023 eligible patients, 835 (81.6%) were defined as IKZF1not-del, 94 (9.2%) as IKZF1del and 94 (9.2%) as IKZF1plus. Of them, 100 (9.8%) corresponded to Standard-Risk (SRG), 629 (61.5%) to Intermediate-Risk (IRG) and 294 (28.7%) to High-Risk (HRG) groups. LFSp(SE) was 7 5(2)% for IKZF1not-del, 51 (6)% for IKZF1del and 48 (6)% for IKZF1plus (p-value < 0.00001). LFSp(SE) according to the risk groups was: in SRG, 91 (4)% for IKZF1not-del, 50 (35)% IKZF1del and 100% IKZF1plus (p-value = ns); in IRG, 77 (2)% IKZF1not-del, 61 (10)% IKZF1del and 54 (7)% IKZF1plus (p-value = 0.0005) and in HRG, 61 (4)% IKZF1not-del, 38 (8)% IKZF1del and 35 (9)% IKZF1plus (p-value = 0.0102). The IKZF1 status defines a population of patients with a poor outcome, mainly in IRG. No differences were observed between IKZF1del versus IKZF1plus. MLPA studies should be incorporated into the risk-group stratification of pediatric ALL.

Children with Down syndrome (DS) are at an increased risk of developing clonal myeloproliferative disorders. The balance between treatment intensity and treatment-related toxicity has not yet been defined. We analyzed this population to identify risk factors and optimal treatment. This single-center retrospective study included 78 DS patients <16 years-old with Transient Abnormal Myelopoiesis (TAM, n = 25), Acute Myeloblastic Leukemia (DS-AML, n = 41) of which 35 had classical Myeloid Leukemia associated with DS (ML-DS) with megakaryoblastic immunophenotype (AMKL) and 6 sporadic DS-AML (non-AMKL). Patients with DS-AML were treated according to four BFM-based protocols. Classical ML-DS vs. non-DS-AMKL were compared and the outcome of ML-DS was analyzed according to treatment intensity. Only four patients with TAM required cytoreduction with a 5-year Event-Free Survival probability (EFSp) of 74.4 (±9.1)%. DS-AML treatment-related deaths were due to infections, with a 5-year EFSp of 60.6 (±8.2)%. Megakaryoblastic immunophenotype was the strongest good-prognostic factor in univariate and multivariate analysis (p = 0.000). When compared ML-DS with non-DS-AMKL, a better outcome was associated with a lower relapse rate (p = 0.0002). Analysis of administered treatment was done on 32/33 ML-DS patients who achieved CR according to receiving or not high-dose ARA-C block (HDARA-C), and no difference in 5-year EFSp was observed (p = 0.172). TAM rarely required treatment and when severe manifestations occurred, early intervention was effective. DS-AML good outcome was associated with AMKL with a low relapse-rate. Even if treatment-related mortality is still high, our data do not support the omission of HDARA-C in ML-DS since we observed a trend to detect a higher relapse rate in the arm without HDARA-C.

Existen numerosas evidencias de los beneficios de la intervención logopédica en pacientes con afasia; no obstante, aún se continúa investigando qué factores aumentan la eficacia de un tratamiento sobre otro. Revisando la bibliografía existente, se ha constatado que uno de los métodos por los que la comunidad científica se está interesando recientemente es la terapia intensiva, que se basa en aumentar considerablemente las horas dedicadas a la intervención logopédica. El objetivo de este Trabajo de Fin de Grado ha sido actualizar los conceptos y evidencias sobre la intervención logopédica intensiva en pacientes con afasia. Para ello, se ha realizado una revisión bibliográfica de ensayos clínicos y otras publicaciones relacionadas.En primer lugar, se ha comprobado que existe un número considerable de investigaciones referentes a esta temática que persiguen objetivos diversos, conformando un complejo y controvertido entramado de evidencias. Los ensayos clínicos revisados, en su mayoría, encuentran eficaces las terapias intensivas; sin embargo, se está investigando acerca de la optimización de los resultados y de los factores que influyen en éxito de la terapia.Por un lado, uno de los temas centrales de investigación es la determinación de la dosis óptima de intervención; es decir, el número de horas (semanales, mensuales y totales) suficientes para alcanzar unos resultados considerablemente más beneficiosos que un régimen regular. Determinar esta dosis es importante teniendo en cuenta que la elevada intensidad del tratamiento conlleva ciertas desventajas como el coste económico o el cansancio de los pacientes. Aunque continúa investigándose, recientes estudios indican que un régimen de entre 4 y 7 horas semanales prolongado durante más de dos semanas podría ser la dosis óptima, sin conseguir mejoras sustanciales aplicando una dosis aún más intensiva de 15 o 20 horas semanales.(AU)

Acute leukemias are the most common neoplasm in pediatric patients. Currently, 80% of children with diagnosis of acute lymphoblastic leukemia (ALL) are cured with conventional chemotherapy, but 20% of them will have a recurrence of the disease. Measurable Residual Disease (MRD) has been described as an important prognostic factor that allows evaluating the response of patients to treatment. One of the most sensitive techniques to study MRD is the quantification of immunoglobulins (Ig) and T-lymphocyte receptors (TCR) genes rearrangements. The aims of this study were to describe the detected Ig/TCR rearrangements, to evaluate the prognostic impact of MRD in our population of children with ALL and to compare the MRD values by Ig/TCR with those obtained by multiparametric flow cytometry (MFC). A total of 455 patients were studied. In 96% of the cases, it was possible to characterize at least one Ig/TCR rearrangement. The total number of Ig/TCR rearrangements detected was 1550. MRD was successfully applied in 89% of the cases. The combined positive MRD values at day 33 and 78 of treatment allow the identification of high-risk patients in cases previously stratified by MRD using flow cytometry at day 15. The comparison between MRD determination by Ig/TCR rearrangements and FC showed excellent correlation. The present work constitutes a study of MRD by Ig/TCR carried out in a very significant number of patients consecutively diagnosed, treated within a homogeneous protocol and with excellent clinical follow-up.


Las leucemias agudas constituyen la neoplasia más frecuente en pacientes pediátricos. Actualmente, el 80% de los niños con leucemia linfoblástica aguda (LLA) logran curarse con quimioterapia convencional pero el 20% de los mismos presentarán una reaparición de la enfermedad. La enfermedad residual medible (ERM) ha sido descripta como un importante factor pronóstico, que permite evaluar la respuesta de los pacientes al tratamiento. Una de las técnicas más sensibles par a estudiar ERM es la cuantificación de reordenamientos génicos de inmunoglobulinas (Ig) y receptores de linfocitos-T (TCR). Los objetivos del presente trabajo fueron describir los reordenamientos detectados de Ig/TCR, evaluar el efecto de la ERM en la supervivencia de niños con LLA y comparar la ERM por Ig/TCR con la cuantificada mediante citometría de flujo multiparamétrica (CFM). Del total de 455 pacientes estudiados, en el 96% fue posible caracterizar al menos un reordenamiento de Ig/TCR. El total de reordenamientos clonales detectados fue de 1550. La ERM pudo ser estudiada en forma exitosa en el 89% de los casos. El valor de ERM positiva combinada al día 33 y 78 de tratamiento, permitió identificar pacientes de alto riesgo, entre los previamente estratificados por la ERM mediante CFM al día 15. La comparación entre la determinación de ERM mediante reordenamientos Ig/TCR y CFM mostró una excelente correlación. El presente trabajo constituye un estudio de ERM mediante Ig/TCR realizado en un número muy significativo de pacientes diagnosticados en forma consecutiva, tratados en el marco de un protocolo homogéneo y con excelente seguimiento clínico.

Resumen Las leucemias agudas constituyen la neoplasia más frecuente en pacientes pediátricos. Actualmente, el 80% de los niños con leucemia linfoblástica aguda (LLA) logran curarse con quimioterapia con vencional pero el 20% de los mismos presentarán una reaparición de la enfermedad. La enfermedad residual medible (ERM) ha sido descripta como un importante factor pronóstico, que permite evaluar la respuesta de los pacientes al tratamiento. Una de las técnicas más sensibles par a estudiar ERM es la cuantificación de reordena mientos génicos de inmunoglobulinas (Ig) y receptores de linfocitos-T (TCR). Los objetivos del presente trabajo fueron describir los reordenamientos detectados de Ig/TCR, evaluar el efecto de la ERM en la supervivencia de niños con LLA y comparar la ERM por Ig/TCR con la cuantificada mediante citometría de flujo multiparamétrica (CFM). Del total de 455 pacientes estudiados, en el 96% fue posible caracterizar al menos un reordenamiento de Ig/TCR. El total de reordenamientos clonales detectados fue de 1550. La ERM pudo ser estudiada en forma exitosa en el 89% de los casos. Abstract Acute leukemias are the most common neoplasm in pediatric patients. Currently, 80% of children with diagnosis of acute lymphoblastic leukemia (ALL) are cured with conventional chemotherapy, but 20% of them will have a recurrence of the disease. Measurable Residual Disease (MRD) has been described as an important prognostic factor that allows evaluating the response of patients to treatment. One of the most sensitive techniques to study MRD is the quantification of immunoglobulins (Ig) and T-lymphocyte receptors (TCR) genes rearrangements. The aims of this study were to describe the detected Ig/TCR rearrangements, to evaluate the prognostic impact of MRD in our population of children with ALL and to compare the MRD values by Ig/TCR with those obtained by multiparametric flow cytometry (MFC). A total of 455 patients were studied. In 96% of the cases, it was possible to characterize at least one Ig/TCR rearrangement. The total number of Ig/TCR rear rangements detected was 1550. MRD was successfully applied in 89% of the cases. The combined positive MRD values at day 33 and 78 of treatment allow the identification of high-risk patients in cases previously stratified by MRD using flow cytometry at day 15. The comparison between MRD determination by Ig/TCR rearrangements and FC showed excellent correlation. The present work constitutes a study of MRD by Ig/TCR carried out in a very significant number of patients consecutively diagnosed, treated within a homogeneous protocol and with excellent clinical follow-up.

BACKGROUND: PCSK9 inhibitors are a treatment option for patients with familial hypercholesterolemia not on low-density lipoprotein cholesterol goals despite the use of maximally tolerated high intensity-statins dose. OBJECTIVE: To evaluate the efficacy of alirocumab and evolocumab in LDL-C reduction and targets attainment in patients with heterozygous familial hypercholesterolemia in clinical practice setting. METHODS: SAFEHEART is an open, long-term prospective study of a cohort of subjects with molecular diagnosis of familial hypercholesterolemia. This study analyze subjects ≥ 20 years of age on stable lipid-lowering therapy, who received PCSK9 inhibitors during the period 2016 to January 2020. RESULTS: 433 patients (mean age 55 years, 53% male, 39% with cardiovascular disease) were included and followed-up for a median of 2.5 years (IQR 1.6-3.0). Median LDL-C level prior to PCSK9 inhibitors was 145 mg/dL (IQR 125-173). The addition of PCSK9 inhibitors (211 alirocumab, 222 evolocumab) reduced LDL-C by 58% (IQR 41-70) p<0.001, in men and women, achieving a median LDL-C level of 62 mg/dL (IQR 44-87) without differences between both PCSK9 inhibitors. Out of them 67% with and 80% without cardiovascular disease reached 2016 ESC/EAS LDL-C targets, and 46% very high risk and 50% high risk patients achieved 2019 ESC/EAS LDL-C goals. Independent predictor factors for attainment of 2019 ESC/EAS LDL-C goals were to be male, smoking and the use of statins with ezetimibe. Both inhibitors were well tolerated. CONCLUSIONS: PCSK9 inhibitors on top of maximum lipid-lowering treatment significantly reduced LDL-C levels in patients with familial hypercholesterolemia and improved the achievement of LDL-C targets.

PURPOSE: The objectives of the study were to establish a procedure for in vivo film-based dosimetry for intraoperative radiotherapy (IORT), evaluate the typical doses delivered to organs at risk, and verify the dose prescription. MATERIALS AND METHODS: In vivo dose measurements were studied using XR-RV3 radiochromic films in 30 patients with breast cancer undergoing IORT using the Axxent® device (Xoft Inc.). The stability of the radiochromic films in the energy ranges used was verified by taking measurements at different depths. The stability of the scanner response was tested, and 5 different calibration curves were constructed for different beam qualities. Six pieces of film were placed in each of the 30 patients. All the pieces were correctly sterilized and checked to ensure that the process did not affect the outcome. All calibration and dose measurements were analyzed using the Radiochromic.com software application. RESULTS: The doses were measured for 30 patients. The doses in contact with the applicator (prescription zone) were 19.8 ± 0.9 Gy. In the skin areas, the doses were as follows: 1-2 cm from the applicator, 1.86 ± 0.77 Gy; 2-5 cm, 0.73 ± 0.14 Gy; and greater than 5 cm, 0.28 ± 0.17 Gy. The dose delivered to the pectoral muscle (tungsten shielding disc) was 0.51 ± 0.27 Gy. CONCLUSIONS: The study demonstrated the viability of XR-RV3 films for in vivo dose measurement in the dose and energy ranges applied in a complex procedure, such as breast IORT. The doses in organs at risk were far below the tolerances for cases such as those studied.

Resumen Durante la ontogenia linfocitaria se produce el reordenamiento de los segmentos génicos V-(D)-J que codifican para la región variable de las cadenas de inmunoglobulinas (Ig) y receptores de linfocitos T (TCR). Durante este proceso, los segmentos se reordenan al azar y ocurren deleciones e inserciones de nucleótidos en la región de unión entre ellos. Los objetivos del presente trabajo fueron describir las incidencias de los reordenamientos Ig/TCR y de los segmentos V-(D)-J involucrados, en niños con leucemia linfoblástica aguda (LLA). Para ello se estudiaron 769 pacientes pediátricos con LLA, diagnosticados entre 1999 y 2018 por los centros de la Sociedad Argentina de Hemato-Oncología Pediátrica. Se caracterizaron reordenamientos de Ig/TCR mediante PCR-multiplex y secuenciación para la búsqueda de recombinaciones génicas IGH, IGK, TCRB, TCRG y TCRD, en muestras de ADN obtenidas de médula ósea o sangre periférica al diagnóstico. El 95% (n=730) de los casos presentaron reordenamientos Ig/TCR. En el 68% de los casos se caracterizaron recombinaciones génicas IGH, en 43% IGK, en 25% TCRB, en 49% TCRG y en el 55% TCRD. Se caracterizó un total de 2506 reordenamientos de Ig/TCR que correspondían 1161 a inmunoglobulinas y 1345 a TCR. En la mayoría de los casos los reordenamientos de IGH fueron completos, IGK involucró a IGKde, TRCB se reordenó frecuentemente con el segmento Jb2, TCRG involucró preferentemente a Vg9 y los TCRD fueron principalmente reordenamientos incompletos. Este trabajo constituye el primer estudio realizado en la Argentina sobre la caracterización de reordenamientos Ig/TCR en un número muy significativo de pacientes con LLA pediátrica.


Abstract During lymphocyte ontogeny, the variable region of immunoglobulin (Ig) and T-cell receptor (TCR) is generated by rearrangements of the V-(D)-J gene segments. In this random process, nucleotide deletions and insertions occur between V-(D)-J segments. The aims of this work were to describe the incidence of Ig/TCR rearrangements, and the V-(D)-J segments involved in acute lymphoblastic leukemia (ALL) patients. With this purpose, 769 pediatric ALL patients belonging to Sociedad Argentina de Hemato-Oncología Pediátrica, diagnosed between 1999 and 2018, were studied. Ig/TCR rearrangements were characterized by multiplex PCR and sequencing to evaluate IGH, IGK, TCRB, TCRG and TCRD rearrangements in DNA samples obtained at diagnosis from bone marrow or peripheral blood. A total of 2506 Ig/TCR rearrangements were characterized, being 1161 immunoglobulins and 1345 TCR. In most cases, IGH rearrangements were complete, IGK involved IGKde, TRCB was frequently rearranged with the Jb2 segment, TCRG preferentially involved Vg9, and TCRDs were mostly incomplete rearrangements. This work is the first study of Ig/TCR rearrangements characterization in a very significant number of childhood ALL carried out in Argentina.

BACKGROUND: Infant acute lymphoblastic leukemia (ALL) is an infrequent disease characterized by clinical and biological features related to poor prognosis. Adapted therapies were designed without a clear consensus regarding the best treatment options. We aimed to compare the outcome between infant ALL cases receiving Interfant versus BFM-based protocols. PROCEDURE: This is a retrospective observational study. From April 1990 to June 2018, infant ALL cases were enrolled in one of the five consecutive treatment protocols. Clinical, demographic, and biological features and outcome were evaluated. A comparative analysis was performed between Interfant protocols and BFM-based protocols. RESULTS: During the studied period, 1913 ALL patients were admitted and 116 (6%) were infants. Treatment administered was: ALL-BFM'90 (n = 16), 1-ALL96-BFM/HPG (n = 7), Interfant-99 (n = 39), Interfant-06 (n = 35), and ALLIC-BFM'2009 (n = 19). The 5-year event-free survival probability (EFSp) was 31.9(standard error [SE] 4.6)% for the entire population, with a significant difference among risk groups according to Interfant-06 criteria (P = .0029). KMT2A-rearrangement status was the strongest prognostic factor (P = .048), independently of the protocol strategy. The median time for relapse was 24.1 months for patients with minimal residual disease (MRD)-negative versus 11.5 months for those with MRD-positive (P = .0386). EFSp and cumulative relapse risk probability (CRRp) were similar. Interfant protocols showed comparable induction (8.1% vs 7.1%, P = .852) and complete remission mortality (21.6% vs 28.6%, P = .438), failing to reduce the relapse rate (48.5% vs 30.7%, P = .149). CONCLUSIONS: Interfant protocols and BFM-based protocols presented comparable results. The risk group stratification proposed by Interfant-06 was validated by our results, and MRD seems useful to identify patients with an increased risk of early relapse.

OBJECTIVE: This study aimed to analyze the prognostic factors for overall and progression-free survival in patients with vulvar cancer. METHODS: This international, multicenter, retrospective study included 2453 patients diagnosed with vulvar cancer at 100 different institutions. Inclusion criteria were institutional review board approval from each collaborating center, pathologic diagnosis of invasive carcinoma of the vulva, and primary treatment performed at the participating center. Patients with intraepithelial neoplasia or primary treatment at non-participating centers were excluded. Global survival analysis and squamous cell histology subanalysis was performed. RESULTS: After excluding patients due to incomplete data entry, 1727 patients treated for vulvar cancer between January 2001 and December 2005 were registered for analysis (1535 squamous, 42 melanomas, 38 Paget's disease and 112 other histologic types). Melanomas had the worse prognosis (p=0.02). In squamous vulvar tumors, independent factors for increase in local recurrence of vulvar cancer were: no prior radiotherapy (p<0.001) or chemotherapy (p=0.006), and for distant recurrence were the number of positive inguinal nodes (p=0.025), and not having undergone lymphadenectomy (p=0.03) or radiotherapy (p<0.001), with a HR of 1.1 (95% CI 1.2 to 1.21), 2.9 (95% CI 1.4 to 6.1), and 3.1 (95% CI 1.7 to 5.7), respectively. Number of positive nodes (p=0.008), FIGO stage (p<0.001), adjuvant chemotherapy (p=0.001), tumor resection margins (p=0.045), and stromal invasion >5 mm (p=0.001) were correlated with poor overall survival, and large case volume (≥9 vs <9 cases per year) correlated with more favorable overall survival (p=0.05). CONCLUSIONS: Advanced patient age, number of positive inguinal lymph nodes, and lack of adjuvant treatment are significantly associated with a higher risk of relapse in patients with squamous cell vulvar cancer. Case volume per treating institution, FIGO stage, and stromal invasion appear to impact overall survival significantly. Future prospective trials are warranted to establish these prognostic factors for vulvar cancer.

MECP2 duplication syndrome (MDS) is an X-linked neurodevelopmental disorder characterized by a severe to profound intellectual disability, early onset hypotonia and diverse psycho-motor and behavioural features. To date, fewer than 200 cases have been published. We report the clinical and molecular characterization of a Spanish MDS cohort that included 19 boys and 2 girls. Clinical suspicions were confirmed by array comparative genomic hybridization and multiplex ligation-dependent probe amplification (MLPA). Using, a custom in-house MLPA assay, we performed a thorough study of the minimal duplicated region, from which we concluded a complete duplication of both MECP2 and IRAK1 was necessary for a correct MDS diagnosis, as patients with partial MECP2 duplications lacked some typical clinical traits present in other MDS patients. In addition, the duplication location may be related to phenotypic severity. This observation may provide a new approach for genotype-phenotype correlations, and thus more personalized genetic counselling.

Objetivos: conocer la etiología más común del daño cerebral adquirido, calcular el porcentaje de pacientes que necesitan neurorrehabilitación y examinar la sintomatología relacionada con el área de logopedia, permitiendo justificar la importancia de este profesional en el ámbito de la rehabilitación neurológica. Pacientes y método: en este estudio se realizó un análisis de los pacientes con alteraciones neurológicas ingresados en un hospital de media estancia, entre el 1 de enero de 2011 y el 1 de enero del 2017. Todos los pacientes eran mayores de 18 años, de ambos sexos, con daño cerebral adquirido. La muestra fue obtenida a través del software de gestión asistencial del Centro Hospitalario Benito Menni de Valladolid, seleccionando las unidades dedicadas a la rehabilitación: convalecencia y daño cerebral, leyendo cada historia para conocer las secuelas y la rehabilitación recibida. Resultados: del total de pacientes ingresados en este periodo 480 presentaban alteraciones neurológicas, de los cuales 39 fallecieron antes de recibir rehabilitación por su grave estado, y 10 empeoraron debiendo cesar las terapias. Finalmente, un 89.7% recibieron rehabilitación, presentando trastornos motores relacionados con los miembros superiores e inferiores en un 90.8%, un 39.8% problemas deglutorios y las alteraciones de la comunicación en un 39.6% de la muestra. La etiología del problema neurológico más común, con un 56.5%, fue el ictus isquémico, seguido del hemorrágico en un 16.9% de los casos y de los traumatismos craneoencefálicos en un 15.6%. Conclusión: la etiología más relevante dentro de un centro de media estancia es el ictus isquémico. Las secuelas más significativas son los trastornos motores, seguidos de los problemas deglutorios y de comunicación. El logopeda es un profesional destacado en la rehabilitación del paciente neurológico, debido a los porcentajes y relevancia de los síntomas tratados desde esta área. Es necesario establecer un método de evaluación de los pacientes con daño cerebral, fiable, estandarizado y validado


Objectives: to know the most common etiology of acquired brain damage, calculate the percentage of patients who need neurorehabilitation and examine the symptoms related to the area of speech therapy allowing to justify the importance of this professional in the field of neurological rehabilitation. Patients and method: in this study, an analysis of the patients with neurological alterations admitted to a half-stay hospital was carried out between January 1, 2011 and January 1, 2017. All patients were over 18 years old, of both sexes, with acquired brain damage. The sample was obtained through the care management software of the Centro Hospitalario Benito Menni in Valladolid, selecting the units active in rehabilitation: convalescence and brain damage, reading each story to know the sequelae and the rehabilitation received. Results: of the total number of patients admitted in this period, 480 presented neurological alterations, of which 39 patients died before receiving rehabilitation due to their serious condition, and 10 patients worsened due to the cessation of therapies. Finally, 89.7% received rehabilitation, presenting motor disorders related to the upper and lower limbs in 90.8%, 39.8% swallowing problems and communication alterations in 39.6% of the sample. The etiology of the most common neurological problem with 56.5% was ischemic stroke, followed by hemorrhagic in 16.9% of the cases and traumatic brain injuries in 15.6%. Conclusion: ischemic stroke is the most relevant etiology within a half-stay center. The most significant sequelae are motor disorders, followed by swallowing and communication problems. The speech therapist is a prominent professional in the rehabilitation of the neurological patient, due to the percentages and relevance of the symptoms treated in this area. It is necessary to establish a method of evaluating patients with brain damage, reliable, standardized and validated

Genetic abnormalities provide vital diagnostic and prognostic information in pediatric acute lymphoblastic leukemia (ALL) and are increasingly used to assign patients to risk groups. We recently proposed a novel classifier based on the copy-number alteration (CNA) profile of the 8 most commonly deleted genes in B-cell precursor ALL. This classifier defined 3 CNA subgroups in consecutive UK trials and was able to discriminate patients with intermediate-risk cytogenetics. In this study, we sought to validate the United Kingdom ALL (UKALL)-CNA classifier and reevaluate the interaction with cytogenetic risk groups using individual patient data from 3239 cases collected from 12 groups within the International BFM Study Group. The classifier was validated and defined 3 risk groups with distinct event-free survival (EFS) rates: good (88%), intermediate (76%), and poor (68%) (P < .001). There was no evidence of heterogeneity, even within trials that used minimal residual disease to guide therapy. By integrating CNA and cytogenetic data, we replicated our original key observation that patients with intermediate-risk cytogenetics can be stratified into 2 prognostic subgroups. Group A had an EFS rate of 86% (similar to patients with good-risk cytogenetics), while group B patients had a significantly inferior rate (73%, P < .001). Finally, we revised the overall genetic classification by defining 4 risk groups with distinct EFS rates: very good (91%), good (81%), intermediate (73%), and poor (54%), P < .001. In conclusion, the UKALL-CNA classifier is a robust prognostic tool that can be deployed in different trial settings and used to refine established cytogenetic risk groups.
